focused on the most important challenges of Croatia\u2019s and Bosnia and Herzegovina\u2019s cross-border area regarding social and healthcare. Inefficient network of healthcare institutions, insufficient focus on quality standards and reduced access to health and social services, especially for people living in rural areas, has motivated the partners to develop an integrated model of care which would improve the social and health care sector. The main objective of PALL NET is to enhance accessibility and quality of health and social services \u00a0provided to elderly, infirm and people with disabilities across the borders. More precisely, the partner will implement social services for persons with disabilities (e.g. mobile teams of support), communication and training activities, in cooperation with other partners. \u00a0<\/span><\/p><p><a href=\"https:\/\/www.dzvk.hr\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Public Health Centre Vinkovci<\/span><\/a> <span style=\"font-weight: 400;\">(CRO) is in charge of implementing the Palliative Care System.
