Start of Mobile Teams within PALL NET project

In accordance with the timeline of activities of the PALL NET project, mobile teams started with their work on 1 December2017: mobile team for people with disabilities, old and infirm, conducted by UOSI Bubamara, Association of Persons with Disabilities, and mobile teams for palliative patients as part of the partners work of the Vinkovci Health Center and Zupanja Health Center.
In order to better present their work and invite the public to participate, PALL NET wants to present the way of work, members of mobile teams and their activities.
A mobile team for people with disabilities, old and infirm, consists of a social worker, physiotherapist and mobile team nurse. The social worker visits the users in their homes and, in a conversation with the users and family members collects information on housing, living and other conditions and needs of the users and family members, organizes visits accordance with their needs and plans further steps to improve the living conditions and relationships within the family .
The mobile team nurse at home of the users cares about the cleanliness, care and everyday needs of the users and family members, while physical therapist performs physical therapy at homes of the users taking into account the diagnosis and degree of disability, as well as the recommendation of a social worker, who is a member of the mobile team.
Today, an increasing number of people suffer from incurable diseases, and spend their last days at home with their family. Palliative care enables them to live the rest of their life without pain and suffering. A mobile palliative team of the Vinkovci Health Center and Zupanja Health Center was established within the PALL NET project. The team started working on 1 December2017, and currently has more than 30 patients. In addition to providing day-to-day support, assistance and education to patients, the team also provides help and support to their family members during illness and after the patient’s death.
